Please understand that our services are only provided with a physicians order. Therefore, you must see a physician and have an order in hand proir to us scheduling an appointment for you.

If you have an upcoming appoinment please fill out the required paperwork here. This will save time for all of us at the time of your appointment. However, if you are unable to complete it online we are happy to assist you in office. You will also be singing the forms and agreeing to our privacy policy which can be downloaded here.
